Motor Claim

At the accident scene: ​
  • Obtain details of all vehicles involved: Year, Make, Model, Colour, License Plate Number, Copy of Insurance Certificate.
  • Identity and contact information for drivers and owners:  name, address, contact number, copy drivers license for driver and owner
  • Details of all persons injured and the damage sustained to the vehicle(s) involved.
  • Witnesses or Passengers: name, address, contact number
  • Take wide angle photos at the scene before vehicles are removed. Include all views (360).  Take photo of damage to your vehicle and any damage to the other party's vehicle or property.
  • Notify the police within 24 hrs or call them to the scene. Collect police reference slip and details of reporting officer name , badge number and police station.
Report accident to your insurer within 30 days and submit the following:
  • Completed claim form
  • Two estimate of repairs
  • Photos of accident scene and damage to vehicle or property
  • Details of any bodily injury
  • Copies of Drivers Licenses for driver and owner. Name, address, contact numbers for all involved parties and any witnesses if available.
  • Copy of Bill of Sale and Road Worthiness Certificate, and Insurance Certificate.
  • Police Report
  • Provide any additional information requested by insurer

Health Claim

Local in Turks & Caicos
  • Complete a Health claim form. The physician must state diagnosis, sign, stamp and date the form. Any associated prescription issued must also be on the medical claim form.
  • Submit original claim form along with original receipts of payments and billing invoices
  • Submit original medical treatment record and or medical report if appropriate and available. (especially for vision and dental)
  • Submit copy prescription and pharmacists prescription receipt, along with prescription payment receipt (originals as appropriate)
Overseas Non-Emergency
  • Obtain necessary pre-approval (mandatory)
  • Submit PMAR referral form from your physician
  • After services rendered submit all the following
  • Original Medical treatment record/medical report, along with original receipts of payments and billing invoices
  • Doctors documents must state any prescriptions, medical aids required, tests etc. in addition to diagnosis. documents (in English)
  • Pharmacists prescription receipt, along with prescription payment receipt (originals required).
  • Original receipts for laboratory tests and supporting documents as necessary
OTHER CLAIMS
  • Contact/ Notify us within 30 days of incident/ loss, or upon becoming aware of any information that could result in a claim against you.
  • Complete and submit a claim form along with copy of the Insured's identification or appropriate signing officer, if it is a company along with a copy business license.
  • Submit photos of damage and any original purchase invoices available
  • Submit detailed estimate of repairs
  • Submit a police report where applicable (theft, burglary, fidelity guarantee, loss of money, certain liability claims)
